It's just a quiz under the guise of a questionnaire.
Spelling isn't too bad, grammer and natural rythm could use a little bit of tweeking. Using synonyms ("spelling!") of "quiz" or other short names instead of a very long "questionnaire". And I think perhaps a longer game next time, instead of a quiz and then nothing.
The colours were also a bit harsh for my liking. Perhaps a more bland blue - or a softer colour.

I haven't played it. Why? It's 10KB in size. 10KB! That's miniscule. The intros to some of my Adrift games have been longer than 10KB. Something that small just can't be any good and, even if it was, it'd be over before you even had chance to enjoy it.
Try uploading a full size game next time.

No it doesn't It just says press enter, then just stops. Does nothing. You can go back through the quiz again, or lose, but that's it.

The introduction to The Shadow Project was about 100k. Yes, I'm still working on it. It's somewhere around 250k now and a lot longer. I've also fixed up basically all of the bugs in my demo version that none of you saw (fortunately - it was horrible). Although the demo didn't quite get you through the introduction and only took about ten or fifteen minutes to finish.

I hope you aren't serious. An intro shouldn't be more than 3 pages.
If it is, then you should consider making it part of the playable game - and even consider splitting the game into parts.
If you consider instructions etc as an intro - don't. If you consider setting variables as part of the intro - don't.
It should definately not take 15 minutes to read through all that and 'get into' the game, that's way too long and people will go within 6.

No, no. The introduction is playable. It's just the portion of the game before the plot is completely revealed and you know what is happening.

Making the player type the word "enter" should be corrected. There is an option in QDK where the player simply hits any key to continue.
Further, for a game (especially a first game) this small, it should not be made into a CAS file. I would be a little further interested in playing if I can find out on my own exactly what commands I am supposed to use to further the game.
For example, someone asked about your van sequence. You give the choices, "talk to the men" or "Jump on the van". Whichever choice the player makes, they have to type out exactly one those options. It's a little much, I think. But in order to get out of the van, the command is 'exit van' and another similar command returns an error. THEN when 'exit van' is executed, you see a note on the paddle. Any command referenced the note or the paddle returns an error.
If at any point in this sequence, the player types 'look', returns you back to the screen where you have the choices of 'talk to the men' or 'jump on the van'. That clearly is an error. Obviously, if you were already in the van and you decided to look while inside the van, why would it put you back deciding whether or not to get in the van.
